Forms and Applications

For PDF forms, you must have Acrobat Reader installed on your computer for viewing, printing, and editing, or commercial Adobe Acrobat or another PDF program.

After downloading, the form can be printed and filled in manually, or some of its sections can be filled in on a computer, or saved in the "save as" mode.


Dear students, please fill in the application legibly and completely - including filling in the year of study, application date, signature, etc.

General forms

Information on enrolling in courses.

The document also contains basic information about student ID cards, the Student Information System, timetable, study system, credits,... and many other information that might be useful at the beginning of your studies and later on.


  • General Application

  • Application for Interruption of Study

    The study will be interrupted at the earliest from the date the dean's decision to interrupt the study comes into force. However, if a student wishes to have his/her studies interrupted as soon as possible, or from a specific date, he/she should bring a written request in person (preferably well in advance) to the Study Department. In the event that the Dean is present and decides on the student's request, the decision can be issued without delay, the student will check it and sign it, if appropriate, waiving the right to review the decision.

  • Notice of Withdrawal from Study

  • Application for the recognition of examinations and course credits

    Students re-entering the PTF and applying for recognition of their previous studies (or individual courses) at the PTF do not need to complete a form and ask the lecturer to recognise the courses individually (see below for SPP students). They can only submit a loosely worded request for recognition and attach a transcript of the courses from SIS as proof, which will be issued and certified as correct by the Study Department.

  • Application to cancel registration for the date of a state final examination

  • Report of Partial Examination

    For all study programmes (continuous and preparatory, comprehensive, rigorous and doctoral). Final examination protocols (Bc. and NMgr. studies) are carried out exclusively through SIS.
